Participant Information Sheet

Participant Information Sheet (PIS)

The project must be accompanied by the Participant Information Sheet addressed to the patient or participant or parent/guardian, in case of minor. While formulating the participant information sheet, investigator must provide the subjects with the following information in a simple understandable layman’s language in English and Hindi, which can be understood by them:

  1. Title of the Study/Project
  2. Aims and methods of the research.
  3. Expected duration of the subject participation.
  4. The benefits to be expected from the research to the subject or to others.
  5. Any risk to the subject associated with the study.
  6. Maintenance of confidentiality of records.
  7. Provision of free treatment for research related injury.
  8. Compensation of subjects for disability or death resulting from such injury.
  9. Freedom of individual to participate and to withdraw from research at any time without penalty or loss of benefits to which the subject would otherwise be entitled.
  10. Amount of blood sample in quantity, in Tea Spoon Full, to be taken should be mentioned.
  11. Costs and source of investigations, disposables, implants and drugs / contrast media must be mentioned.
  12. Telephone number/contact number of Principle Investigator and Co investigator at the top of each page. In case of drug trials:
    1. The chemical name of the drug, date of its manufacturing and batch number must be mentioned
    2. Initial Bio equivalent study of the drug / references should be provided
  13. Self certification should be given that translation to vernacular is accurate.

Mrs. Shalini Verma, W/o Naveen Gupta, Hospital Attendant – III, RZG-66, Gali Som Bazar Wali, Mahavir Enclave, Palam Road, New Delhi – 110045 is hereby informed that she has neither attended the inquiry proceedings to defend her case of unauthorized absence from her duty w.e.f. 19.05.06 nor sent any communication in this regard in spite of several letters dispatched to her known addresses. She is again directed to appear before the undersigned within seven days from the publication of the notice in the newspaper, failing which the case will be decided ex-pârté.

ALL CANDIDATES WHO APPEARED FOR THE SAID INTERVIEW ARE NOW INFORMED THAT THEY WILL BE GIVEN AN OPPORTUNITY TO APPEAR IN A WRITTEN/OBJECTIVE EXAMINATION FOLLOWED BY AN INTERVIEW.

THE WRITTEN EXAMINATION IS SCHEDULED FOR 09.03.2008 AND ACCORDINGLY ADMIT CARDS WILL BE ISSUED TO ALL.
